COMMENTS  

The contract for Project Reclaim Phase II was signed on 12/28/21 and a Notice to Proceed was issued the same day.  
Work at the site was already on going from Project Reclaim Phase I.  Material haulage transitioned from Phase I to Phase 
II on the day the contract was signed.   Material haulage continues daily on the project.  Removal of fines is nearly 
complete as well as construction of the retaining pond at the lower end of the site.  Berms around the structure have been 
covered with a layer of topsoil.   
 
Application for Payment #6 requested payment in the amount of $220,130.44.  Payment of the invoice was 
recommended.    Photos of the site are shown below.
